Las mujeres son muy propensas a padecer infecciones urinarias, en particular, de cistitis. El motivo reside en que la uretra femenina es mucho más corta que la masculina, lo que facilita la migración de gérmenes de la zona vaginal y perineal a la vejiga. En las pacientes jóvenes, las relaciones sexuales son una de las principales causas y en la mujer de más edad, la menopausia y los cambios vaginales facilitan la colonización de bacterias. Una de las infecciones más características del hombre es la infección con inflamación de la próstata o prostatitis.
En el hombre, la longitud de la uretra y la próstata con sus secreciones específicas, actúan de barrera frente a la invasión de gérmenes. Es muy poco frecuente antes de la adolescencia y es la infección urinaria más frecuente hasta los 50 años. A partir de esa edad son las patologías prostáticas, como la hiperplasia benigna de próstata (el aumento de la próstata con el envejecimiento dificulta el vaciamiento correcto de la orina), la biopsia de próstata o la estenosis de uretra, las que incrementan el riesgo de infección. El origen de este cuadro infeccioso agudo suele deberse al reflujo por vía retrógrada de gérmenes desde la uretra o la vejiga hacia la próstata.
La prostatitis aguda se caracteriza por la aparición súbita de dolor en la zona del periné (situado entre el ano y los testículos), y puede acompañarse de molestias al orinar con sensación de ardor en el trayecto de la uretra y aumento de la frecuencia miccional. Si el paciente tuviera fiebre, requiere muchas veces ingreso hospitalario.
El tratamiento se basa principalmente en antibióticos que deben ser tomados durante varias semanas, para evitar que se acantonen colonias de bacterias en la próstata y un cuadro agudo se convierta en crónico.
